Designation Clinical Coordinator - 1 Opening(s) Job Description 1. To counsel patients on the renal replacement therapy options. 2. Train the patient/care giver on the process of peritoneal dialysis, infection-identification and immediate management. 3. Visit patients for follow-up and undertake a complete recall of the patients compliance to earlier instructions. 4. Observe the exchange procedure being undertaken by the care taker/patient. 5. Guide the patient/care taker through the entire process of peritoneal dialysis exchange including room set-up, hand-washing, exit site care and dressing, and actual exchange. 6. Prepare a detailed note on the observations including biochemistry reports and share with the patient/care giver and Clinical Team Leader. 7. Prepare a list of patients who can be moved to Extraneal/APD. 8. Plan for patient file discussion with Drs. 9.
