1 Take actual complete history of the patient and perform thorough clinical examination immediately on admission and inform the status to admitting and treating consultant. Current medicine taken by the patient to be listed and entered in the file.
2 To write progress/clinical notes including vitals, I/O etc, with date and time everyday with his/her signature & name at least twice, once on arrival to the duty and second before leaving the duty. In case of critical cases the clinical notes has to be frequent.
3 Take detailed clinical rounds of the patient in ward /ICU before consultant visits and later accompany the consultant during their visit, and inform consultant regarding any new development related to the patient.
4 To attend emergency promptly and inform consultant immediately. Provide immediate management as per the instruction from the consultant. Attend the patient until consultant arrives or till patients condition stabilized. Inform in-house registrar if required.
5 To check that the treatment & investigations are being carried out as per the consultant's order.
6 To ensure that Intra venous injection and drips are being administered under supervision. To ensure that Medication are administered after checking proper dosage and route of administration.
7 In case of any cross-reference, inform the consultant and make sure that it has been seen by the respective consultant in time and remind for the same if necessary. Inform primary consultant regarding advice given by them.
8 Before patient gets discharged, discharge summary is to be written in proper and check the discharge summary typed by MT before signing, for correct spelling, dose, duration, route to administer the drug etc.
9 To give and take detailed over from Medical Officer in writing.
Administrative Duties
1 Explain the relatives about patient's condition politely & patiently.
2 Talk to patient frequently to make them feel that they are being taken care of and also enquire about any problems faced related to hospital services etc. Resolves the issues if any.
3 Booking of O.T. if required./ requested by the consultant. During OT booking ensure that the additional information related to surgery is also noted and informed to the OT staff as well as surgeon regarding the OT number, date and time of surgery.
4 To arrange for blood, if required.
5 To fill OT clearance form completely and correctly after communicating with consultant & to send the patient to OT after clearance of all dues. Confirm & ensure signature of patient and consultant in OT form.
6 To ensure completion of medical clearance procedure and ensure that all the abnormal findings are informed to treating consultant and anesthetist.
7 Ensure that informed consent form is completely (risk, benefit, procedure and other alternative option) filled and explained thoroughly by consultant. Signature of consultant is mandatory.
8 To ensure completion of MRD file documentation related to history
9 If insurance/cashless patient Medical officer has to fill up pre authorization form and give estimate in consultation with treating consultation also help mediclaim desk in any insurance related queries.
10 Consultant visit has to be duly signed and authorized by the Medical officer.
11 It is mandatory to fill and sign BT form. Radiology, blood request and pathology request form.
