Job Title: Clinical Coordinator

Job Description:

·         Provide efficient and high quality training and support to existing dialysis patients by visiting them at home. The Patient follow up by the Clinical Coordinator will include the following:

·         Every patient should have a baseline report filled immediately after catheter insertion

·         Every patient will be visited by a Clinical Coordinator at least once a month on topmost priority.

·         The Clinical Coordinator will diligently fill the patient follow up sheet after probing the patients for their well being. The Clinical Coordinator will examine the patient for their physical signs and symptoms of malnutrition, fluid overload, anaemia, level of activity, depression and record them in the follow up sheet. The Clinical Coordinator will use the HRA chart to assess the risk status of the patient.

·         The Clinical Coordinator will be responsible for monitoring bag sale index and assess the patients for their compliance on the therapy.

·         The Clinical Coordinator will consult the Clinical Team Leader or the Area Clinical Coordinator and the Nephrologist in managing complications.

·         The Clinical Coordinator reports patient coverage and losses in their weekly report every Monday to the Clinical Team Leader and the KAM. Clinical Coordinators are accountable for loss reporting
